Xanthelasma is a common condition characterised by soft, yellowish cholesterol deposits that develop on or around the eyelids. Although these deposits are harmless and usually painless, they can become cosmetically bothersome and may sometimes indicate underlying cholesterol or lipid abnormalities.

What are the Symptoms?
- Yellow or cream-coloured flat plaques on the upper or lower eyelids
- Soft, painless deposits near the inner corners of the eyes
- Gradual increase in size over time
- Usually affects both eyes (bilateral)
- Cosmetic concern due to visible eyelid lesions
When Do You Need Xanthelasma Removal?
Treatment is generally recommended when:
- The lesions are cosmetically noticeable
- The plaques continue to enlarge
- They interfere with the natural appearance of the eyelids
- Previous treatments have failed or the lesions have recurred
Although xanthelasma is not cancerous and does not affect vision directly, many patients choose treatment to improve facial aesthetics and confidence.
Procedure
At MEPS, xanthelasma removal is performed as a day-care procedure under local anaesthesia.
Depending on the size and depth of the deposits, treatment may involve:
- Surgical excision
- Radiofrequency removal
- Laser treatment (selected cases)
When surgery is required, the incision is carefully planned within the natural eyelid creases whenever possible to minimise visible scarring. The cholesterol deposits are completely removed while preserving healthy surrounding tissue. Fine sutures are used and are generally removed within one week.
The procedure typically takes 30–60 minutes, depending on the number and size of lesions.
What to expect?
Following treatment, mild swelling, redness and bruising around the eyelids are common and usually resolve within a few days.
Patients are advised to:
- Apply prescribed antibiotic ointment
- Keep the area clean and dry
- Avoid rubbing the eyes
- Protect the skin from excessive sunlight
- Attend follow-up appointments for wound assessment
Although the deposits are removed successfully, recurrence is possible, especially if underlying cholesterol levels remain uncontrolled.
Time for recovery
Most patients recover quickly after xanthelasma removal.
- Mild swelling lasts 3–5 days.
- Stitches, if used, are removed within 5–7 days.
- Most patients resume routine activities within 2–3 days.
- Complete healing generally occurs over 2–3 weeks.
- Scar maturation continues over the following few months.
